Job Summary
A healthcare company has a current position open for a Remote Care Management Intake Coordinator.
Individual must be able to fulfill the following responsibilities:
- Provide initial screening of potential complex case management referrals
- Work with Case Managers to insure potential case management referrals are appropriate for the program
- Research and provide clinical and complex resource information for case managers
Skills and Requirements Include:
- Bachelor's in social services, human services, behavioral sciences or licensed in allied health field
- 3 years' clinical experience or healthcare experience
- 5 years clinical experience with patients
- 3 years healthcare experience
- Prior Mental Health/Social Work experience
