Remote Care Manager RN

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and develop personalized care plans
• Coordinate care between members, families, and healthcare providers
• Monitor member progress and revise care plans as necessary

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Preferred: 2+ years of clinical nursing experience in acute care settings
• Preferred: 2+ years of case management experience in a managed care environment
