Job Summary
A healthcare products provider has a current position open for a Remote Case Manager II.
Candidates will be responsible for the following:
- Researching and identifying available community and network resources
- Conducting on-site visits to facilities or Member's homes
- Using established criteria, analyzes and evaluates referrals
Position Requirements Include:
- Attends company and departmental meetings as required and travel to provider and member locations
- Ability to operate a personal computer
- Knowledge of URAC and NCQA standards for case management organizations
- RN or Social Worker with an active PA license and 3 yrs recent clinical experience;or health care professional with a BS (or higher) in social work.
- Knowledge of managed care principles and emerging health treatment modalities
- Possession of a valid driver's license
