Job Summary

A health insurance company is filling a position for a Remote Certified Diabetic Educator Telephonic Care Manager.

Must be able to:

  • Manage members' physical, environmental and psycho-social health issues
  • Resolve barriers that hinder effective care
  • Ensures patients are progressing towards desired outcomes

Skills and Requirements Include:

  • Travel will be no more than 10% to market offices
  • Must have a separate room with a locked door that can be used as a home office
  • Licensed Registered Nurse (RN) in the state of Florida with no disciplinary action
  • 3 - 5 years of adult clinical acute care experience
  • 1 year experience Certified, Diabetic Educator, CDE within the adult population
  • Comprehensive knowledge of Microsoft Office applications
