Job Summary
A healthcare company is in need of a Remote Clinical Care Management Coordinator .
Must be able to:
- Engage facility contacts through effective relationship management
- Review medical record for new patients in order to prepare information as needed
- Review facility census and identify patients who meet criteria for the Chronic Care Management program
Qualifications for this position include:
- If necessary, travel to local nursing homes to assist with reviewing medical records
- Expected to be available via telephone during business hours
- Expected to maintain great customer service
- Demonstrated capability of working in a remote environment with limited face-to-face interaction with supervisor
