Job Summary
A health insurance company is filling a position for a Remote Community Transitions RN Case Manager.
Individual must be able to fulfill the following responsibilities:
- Complete post discharge assessments evaluating members
- Follow model of care requirements as outlined in policies
- Educate members on disease process and medications
Position Requirements Include:
- Current unrestricted RN License in a compact state of residence
- 2+ years case management, managed care, home health experience OR acute care clinical experience
- Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
- Internet access
- Must have a designated workspace and access to install secure high speed internet via cable in home
