Job Summary

Amental and behavioral health support organization needs applicants for an opening for a Remote Direct Support Professionals Caregiver in Baraboo.

Individual must be able to fulfill the following responsibilities:

• Provide and coordinate direct services and healthful lifestyle support to individuals with varying mental and physical disabilities
• Assist the individuals in every aspect of their lives according to their Individualized Support Plans (ISP)
• Assist in maintaining a clean and safe living environment and meal preparation

Applicants must meet the following qualifications:

• Must be at least 18 years old
• Valid driver's license with acceptable driving record
• Reliable transportation
• Current auto liability insurance
• Acceptable criminal record
