Job Summary

We have an immediate need for several Registered Nurses to make field assessment visits throughout downstate New York and complete appropriate documentation to determine members' enrollment, service needs, and continued plan eligibility as required and defined by the Department of Health.

This is a salaried position with benefits, and an outcomes based bonus incentive plan. There is a two week paid orientation and training period, followed by remote field assessments and administrative follow-up from your home.

You must be willing and able to travel throughout the five boroughs with occasional travel to Long Island. There is an expectation of two to three reviews daily; mileage is reimbursed between the first and last reviews of the day.


Qualification
Unexpired NY state drivers' license or NY state ID
You must be eligible to work for any employer in the US without need of sponsorship.
Work experience in one or more of the following areas preferred but not required:
-Geriatrics
-Home Care
-Discharge Planning
-Case Management
-Medical Surgical Nursing

Experience using electronic patient health information (PHI) database. UAS experience a plus!
Intermediate Microsoft Word, Excel, and Outlook skills
Knowledge of health insurance, Medicaid, Medicare and MLTCP
Experience working with adult or elderly population with the ability to determine appropriate care plans and services for frail population, as well as ability to negotiate initial service plans so that members and families are in agreement
Field experience assessing, planning, and evaluating member's/patient's care a plus
Bilingual in the following is a plus: Spanish, Russian, French, Creole, Mandarin, Cantonese

Licensure
Active, unrestricted Registered Nurse license in the state of New York
