Job Summary
A global healthcare company is seeking a Remote Field Based Health & Social Services Clinical Coordinator in Austin.
Core Responsibilities Include:
- Spending about 25% of your time in the field visiting our members in their homes or in long-term care facilities
- The ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices
- Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
Skills and Requirements Include:
- Social worker or Registered Nurse (RN) with an active Texas license
- 4+ years of Clinical experience
- 1+ years of experience working with MS Word, Excel and Outlook
- Great at communication and advocating for best possible care
