Job Summary

A global healthcare company has an open position for a Remote Field Based RN Care Coordinator in North Tidewater.

Individual must be able to fulfill the following responsibilities:

• Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
• Conduct initial and follow-up assessments within designated timeframes
• Identify and initiate referrals for social service programs

Applicants must meet the following qualifications:

• This role requires a minimum of 25 % of time spent working in a field based capacity
• Bachelor’s degree in Social Work or Human Services
• 3+ years of experience working within the community health setting in a health care role
• 2+ years of behavioral health experience
• 1+year experience working with Medicaid population and working with GAP population
• Intermediate skills with MS Word, Excel and Outlook
