Job Summary

A healthcare company is filling a position for a Remote HIV Prevention Case Manager.

Must be able to:

• Ensure your assigned clients receive a coordinated range of client-centered services
• Complete assessments with clients to include completion of a care plan
• Enter all client data, units of service, and case notes into the database

Qualifications for this position include:

• 2 years of paid post-degree experience in direct service to HIV target population
• Exceptional written and verbal communication skills
• Bachelor’s in social work or other related four year degree in social services or human services preferred
