Job Summary

A healthcare support company is in need of a Remote Intensive Support Coordinator in Dougherty County.

Candidates will be responsible for the following:

  • Conducting health, safety, social, and environmental assessments
  • Linking participants with medical, social and education providers
  • Monitoring the health and welfare goals of the participant

Skills and Requirements Include:

  • Ability to travel as needed
  • 2 years experience serving persons with DD/IDD or related
  • Bachelor's in human service related field
  • Experience in healthcare, rehabilitative, behavioral or related health setting
  • Experience in assessments and service plan development/monitoring
  • Willingness and ability to work flexible schedule to meet needs of families
