Job Summary
A healthcare company has an open position for a Remote Kidney Care Central Nurse.
Candidates will be responsible for the following:
- Conducting assessments that include the medical, behavioral, pharmaceutical, and social needs
- Creating and implementing patient care plans to be clearly documented
- Managing patients during periods of transitions of care to facilitate effective transition
Required Skills:
- 3-5 years of nursing experience in case management or care management
- Current, unrestricted compact Registered Nurse license
- Able to work on multiple projects at once and juggling competing priorities
- Effective written and verbal communication skills
- Knowledge and experience to empower patients in self-management
