Job Summary

A healthcare company is in need of a Remote Licensed Care Coordinator.

Individual must be able to fulfill the following responsibilities:

  • Provide care coordination to members with behavioral health conditions
  • Conduct in depth health risk assessment and/or comprehensive needs assessment
  • Communicate and develop the care plan

Must meet the following requirements for consideration:

  • 5 years post-licensure clinical experience
  • Knowledge of various public and private services available for individuals with mental illness
  • Associates Degree in Nursing required for RNs, or Masters Degree in Social Work or Healthcare-related field, with an independent license, for Social Workers
  • Licensed in State that Services are performed
