A chronic healthcare solutions company has an open position for a Remote Licensed Registered Nurse Integrated Clinical Care Management Supervisor.
Core Responsibilities Include:
- Providing clinical direction, organization, and monitoring of all care management services
- Conducting care management screening, care coordination, care management and other supportive functions
Applicants must meet the following qualifications:
- Bachelor’s Degree in Nursing (BSN) or equivalent education and experience
- Registered Nurse with Clear/Active license in the stae of Florida or a multi-state compact license
- 3 to 5 years clinical practical experience
