Job Summary
A non-profit, Catholic-based health system is in need of a Remote LPN Care Coordinator.
Candidates will be responsible for the following:
- Maintain a caseload of patients according to department policies
- Identify, enroll and/or refer patients to Ambulatory Care Manager
- Support execution of developed care plans to maximize healthcare outcomes and facilitate wellness
Required Skills:
- High School Diploma
- Licensure/Certification Required: Licensed Practical or Vocational Nurse with active License in State of Patient Care
- Licensure/Certification Preferred: Community Health Worker certification or working toward certification, Certified Health Coach
- 2 years experience in community based setting, home health or case management experience
- Excellent interpersonal communication and negotiation skills
