Job Summary
A care management provider for newborns is searching for a person to fill their position for a Remote Maternity Program Social Work Case Manager.
Individual must be able to fulfill the following responsibilities:
- Complete initial and ongoing assessments within established timeframes to identify needs and barriers
- Use engagement models and tools to maintain high engagement rates
- Assess, plan, and implement care strategies that are member centric and directed towards meeting the needs of the member
Qualifications Include:
- Master’s degree in Social Work (MSW) required
- 2 years background working in managed care or relevant clinical environment, with maternity population
- Must have valid unrestricted Social Worker license to practice in state of residence & agree to obtain licensure in other state(s)
- CCM certification preferred upon hire; required within the timeframe specified by company policy
- Experience with data entry into a database or EHR required. Must be computer literate
