A healthcare company is seeking a Remote Medical Case Manager in Brick Township.
Candidates will be responsible for the following:
- Handling complete case management of all patient needs
- Interviewing patients and caretakers to assess needs and risk categories
- Collaborating with primary care physicians and the clinical team on plan of care
Must meet the following requirements for consideration:
- Knowledge of health care practices, insurance and billing
- Familiar with the NJ landscape, especially social services within NJ
- Capability to effectively communicate with patients, providers, and family members
- Ability to follow HIPAA guidelines and regulations
- Ability to acknowledge patient’s rights on confidentiality issues
- Ability to work productively, either independently or with other individuals
