Job Summary

A healthcare company is in need of a Remote Medical Case Manager in the Charlotte Area.

Core Responsibilities of this position include:

• Being responsible for case management of eligible members
• Working with members with complex care needs
• Interacting with members in their home on a regular basis

Applicants must meet the following qualifications:

• Ability to travel within a designated geographic area
• RN licensure required
• 3+ years clinical experience required
• 1+ years case management, discharge planning, or home health experience
• Associates degree with equivalent experience
• Obtain a bachelor’s degree within 3-5 years
