Job Summary
A health care organization needs applicants for an opening for a Remote RN PRN Care Facilitator in Salt Lake City.
Candidates will be responsible for the following:
- Reviewing referrals for inpatient behavioral health treatment from internal and external customers
- Assessing and reassess needs of patients through verbal discussions with referring staff
- Interacting with third-party payers to coordinate admissions or transfers as needed
Skills and Requirements Include:
- Minimum of 3 years in a behavioral healthcare setting performing assessments
- Associate's Degree in Nursing
- Must hold and maintain current licensure as a RN license
