A non-profit healthcare system has a current position open for a Remote Social Work Case Manager in Pensacola.
Must be able to:
- Oversee primary needs of patient
- Provide primary home care/consultation to complex client and family needs
- Monitor patient care through assessments, evaluations and/or patient records
Position Requirements Include:
- Licensure required relevant to state in which work is performed
- High school diploma/GED with 2 years of experience, or Associate's or Technical degree
- Ability to advocate for resources and removal of barriers
