Job Summary

A healthcare company has a current position open for a Remote Telephonic Care Navigator.

Candidates will be responsible for the following:

  • Provide timely telephonic follow-up by keeping track of scheduled tasks
  • Maintain and monitor assigned patient roster
  • Effectively utilize Electronic Health Record systems to gather patient medical history

Must meet the following requirements for consideration:

  • Strong Communication Skills
  • Case Management or Care Coordination experience
  • Medical Assistant Front/ Back office experience - 5 years minimum
  • Basic clinical tasks in the offices of physicians, hospitals, and other healthcare facilities
  • Basic typing skills
