Job Summary

A global healthcare company needs applicants for an opening for a Remote Telephonic Community Health Worker.

Candidates will be responsible for the following:

• Create a positive experience and relationship with the member
• Complete New Member process (Welcome Call, Demographic Research, Screenings & scheduling Health Risk Assessment)
• Keep the member out of the hospital by supporting regular visits to their primary physician

Position Requirements Include:

• High School Diploma / GED (or higher)
• Knowledge of culture and values in the community
• Familiarity with the resources available within the community
• Basic Microsoft Excel experience (ability to create, edit, copy, send and save spreadsheets)
• Must have Broadband / high speed internet capability from home
• Certified Nursing Assistant, Home Health Aide or Medical Assistant preferred
