Job Summary
A global healthcare company needs applicants for an opening for a Remote Telephonic Community Health Worker.
Candidates will be responsible for the following:
- Create a positive experience and relationship with the member
- Complete New Member process (Welcome Call, Demographic Research, Screenings & scheduling Health Risk Assessment)
- Keep the member out of the hospital by supporting regular visits to their primary physician
Position Requirements Include:
- High School Diploma / GED (or higher)
- Knowledge of culture and values in the community
- Familiarity with the resources available within the community
- Basic Microsoft Excel experience (ability to create, edit, copy, send and save spreadsheets)
- Must have Broadband / high speed internet capability from home
- Certified Nursing Assistant, Home Health Aide or Medical Assistant preferred
