Job Summary

A healthcare staffing firm is in need of a Remote Telephonic Nurse Case Manager.

Core Responsibilities of this position include:

  • Reviewing data submitted through referral sources, applying the appropriate criteria and standards in screening patients for case management services
  • Completing assessment of identified patients through review of past utilization, discussions with the primary care physician and specialists evaluation of the patient
  • Implementing a plan of care

Skills and Requirements Include:

  • Clear and active LVN/ RN License
  • 2+ years with Telephonic Case Management experience from an Acute Care Setting
  • 2+ years of Adult Managed Care experience: MediCal, Medicare, and Commercial
  • Ability to evaluate patients telephonically
  • Ability to work remotely
