Job Summary

A healthcare company needs applicants for an opening for a Remote Transition Care Coordinator.

Individual must be able to fulfill the following responsibilities:

  • Perform clinical and operational processes
  • Implement, evaluate, and coordinate the plan of care for all assigned patients
  • Develop care plan and other case management tools

Qualifications Include:

  • Current unrestricted Registered Nurse license
  • Must possess multistate license in state of residence
  • Minimum 5 years clinical experience with at least 3 years managed care experience
  • 2 – 4 years’ experience in utilization management
