Core Responsibilities of this position include:
- Developing a proactive plan of care
- Conducting an evaluation of member's needs and benefits
- Appling clinical judgment to incorporate strategies designed to reduce risk factors

- Some travel is required
- Associates required
- 3-5+ years clinical practical experience
- Registered Nurse with active state license in good standing
- Excellent analytical and problem-solving skills
