Job Summary
We have a great telecommute opportunity for a Registered Nurse Case Manager with a background in Transplant Case Management living within the Pacific Time Zone! This is a full-time position, Monday thru Friday during normal business hours.

The Ideal Candidate Will:

Facilitate the clinical progression of care and collaborate with the health plan member and their family, as well as health plan and community providers.
Manage and prioritize care coordination activities utilizing their knowledge of complex care planning.
Utilize decision-making and critical-thinking skills in the review and determination of coverage for medically necessary health care services.
Identify process improvement opportunities.

Qualification
A degree from an accredited program of nursing.
A minimum of four years Case Management or acute care coordination experience; preferably in a Managed Care setting.
A strong transplant background.
Strong computer skills; proficient in Windows, Excel and prior use of a laptop.
US Citizenship and the ability to pass federal security clearance screening.
An active, unrestricted Registered Nurse license in the state of residence
