RN Care Manager
Job Summary
A company is looking for an RN Care Manager for a full-time position.
Key Responsibilities:
- Provide individualized nursing care and patient education
- Coordinate patient care across the continuum by collaborating with healthcare team members
- Act as an advocate for patients and families to coordinate care and support quality improvement
Required Qualifications:
- Bachelor's degree in nursing
- Minimum of three years' experience in population health, case management, or related field
- Unencumbered RN license with the State Board of Nursing
- Care Coordination Transition Manager (CCTM) certification required within 18 months
- Basic Life Support (BLS) certification required within six months of employment
