RN Care Manager

Job Summary

As a fully remote RN Care Manager, the successful candidate will manage a panel of patients, focusing on transitions of care for individuals with kidney disease, while collaborating with healthcare teams to develop and implement comprehensive care plans that address each patient's unique needs.

Key responsibilities

• Managing patient care through assessments and the development of actionable care plans tailored to individual goals and preferences
• Establishing trusting relationships with patients and families to provide emotional support and facilitate collaboration in their care journey
• Coordinating interdisciplinary efforts to ensure continuity of care and effective management of kidney disease progression post-hospitalization

Required qualifications

• Associate Nursing degree with an active RN license (compact license preferred)
• 3+ years of care management experience
• 2+ years of Transitions of Care experience
• Chronic and complex care management experience
• Intermediate skills with MS Office Suite, including Outlook and Teams
