Job Summary
Providing comprehensive coordination of services for service members and their families, the full-time RN Case Manager will manage patient care, documentation pathways, and act as a liaison between military and civilian medical agencies in a remote setting.
Key responsibilities
- Conduct assessments, planning, and evaluation of care to promote optimal health and support transitions back to duty or civilian life
- Coordinate patient care and documentation pathways while ensuring continuity of care across various agencies
- Review and summarize case facts, rendering medical necessity determinations and resolving technical issues within assigned cases
Required qualifications
- Bachelor's degree with 5-7 years of relevant experience, with an advanced degree preferred
- Active RN license with a minimum of a two-year nursing degree from an accredited institution
- At least 2 years of nursing and/or case management experience
- Certification as a case manager by a recognized organization or the ability to obtain certification within one year of hire
- Professional oral and written communication skills with various stakeholders, including Military Health System personnel
