Job Summary
A company is looking for an RN Case Manager - Remote.
Key Responsibilities:
- Meet with patients, families, and caregivers to discuss care and treatment plans
- Consult with the care team to ensure successful implementation of care plans
- Monitor and coach patients using motivational techniques to maximize self-management
Required Qualifications:
- Graduation from an accredited school of nursing
- Active, unrestricted Registered Nurse license through the State of California
- 1+ years of experience in a clinical setting or care management
- Ability to work Monday - Friday, 8-5 PST
- Bachelor of Science in Nursing (BSN) preferred
