Job Summary

A company is looking for a Senior Director of Care Management and Clinical Operations.

Key Responsibilities

• Lead program development and operations management across the care management platform
• Design and implement clinical programs related to specific disease states and clinical priorities
• Measure clinical outcomes and identify improvement opportunities using benchmarking

Required Qualifications, Training, and Education

• Bachelor's degree in nursing or social work with current licensure preferred
• 3+ years of nursing experience in acute, ambulatory, or primary care settings
• 5+ years in care management or value-based care leadership in an ambulatory or managed care setting
• Direct management experience building and leading teams in a fast-growing environment
• Experience managing change through process improvement and transformation
