Job Summary

- Supervise RNs, LVNs, and Health Advocates, focusing on daily operational responsibilities
- Ensure successful execution of projects and timely completion of tasks across the team
- Lead patient education initiatives and oversee team interactions with patients
Required Qualifications
- Graduate from an accredited school of Nursing
- Current California RN License
- Care management experience
- Experience with ACO Commercial and Medicare programs
- Intermediate level of experience with Microsoft Suite
