Job Summary

A provider of behavioral health solutions needs applicants for an opening for a Telecommute Behavioral Health Care Coordinator Case Manager in Texarkana.

Core Responsibilities Include:

  • Communicating with members about their needs at a regular frequency
  • Developing and maintaining the Plan of Care, and making information accessible to providers and members
  • Documenting all activity in the designated system

Required Skills:

  • Ability to provide in-person care; might require some out-side business hours on call rotation
  • GED or High School Diploma required
  • One year experience with developmentally or intellectually disabled and/or complex mental health conditions
  • Access to reliable internet
  • Valid driver’s license and reliable transportation
  • Ability to document within care management system; knowledge of Microsoft Word and Excel
