Job Summary
A health insurance company is searching for a person to fill their position for a Telecommute Behavioral Health Case Manager l in Houston.
Candidates will be responsible for the following:
- Using appropriate screening criteria knowledge and clinical judgment to assess member needs
- Conducting assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment
- Monitoring and evaluating effectiveness of care plan and modifies plan as needed
Qualifications for this position include:
- Must reside within 1 hour of the office and be willing to travel to the local office on a monthly basis
- Requires MS or MA in social work, counseling, or a related behavioral health field
- 3 years clinical experience
- Current active, unrestricted license
