Job Summary

A healthcare software company is seeking a Telecommute Bilingual Spanish Health Care Navigator .

Candidates will be responsible for the following:

  • Providing consultation and referrals to appropriate community resources and providers
  • Collaborating with internal partners in the clinical and coaching departments
  • Prioritizing member satisfaction, outcomes, risk mitigation, and adherence

Applicants must meet the following qualifications:

  • Ability to travel to all-hands meetings, as needed
  • 2+ years experience in a care navigation, referral coordinator, or crisis intervention
  • Licensed in Behavioral Health - LCSW or LMFT
  • Experience in a customer service client-facing role
