Job Summary
A healthcare company has a current position open for a Telecommute Care Coach .
Candidates will be responsible for the following:
- Engaging, enrolling, and maintaining members in active participation in the system
- Coordinating member care
- Providing exemplary customer service to and member-centric focus for, all clients
Must meet the following requirements for consideration:
- Licensed RN/LPN/LVN with a strong interest in behavioral health
- Minimum of three years of direct clinical experience in an acute care setting is required
- Must have excellent written and verbal communication skills
- Demonstrate proficiency in the utilization of Microsoft Office suite of products
