Job Summary

A healthcare company is in need of a Telecommute Care Management Senior Pharmacist.

Must be able to:

  • Develop care plans for individual members and coordinate members' care
  • Assist in the development and implementation of various programs
  • Address gaps in care, medication adherence and perform medication reconciliation

Applicants must meet the following qualifications:

  • Travel required (0% - 25%)
  • Registered Pharmacist
  • Bachelor of Science in Pharmacy or a Doctor of Pharmacy degree
  • 5 - 7 years of post-graduate pharmacy experience
  • Must be licensed in the State of Arkansas
