Job Summary
A Healthcare Company is filling a position for a Telecommute Case Manager Registered Nurse.
Candidates will be responsible for the following:
- Performing assessment, screening, evaluation of patient
- Developing patient-centered care plans
- Maintaining knowledge of case management standards of practice
Qualifications for this position include:
- Registered nurse with an active nursing license
- Three to five years clinical experience
- One to two years of case management or care coordination experience
- Successful work history in a clinical setting
- Knowledge of CPT, ICD-10, and/or HCPC codes or coding experience
