Job Summary

A developer of products for use in healthcare and clinical management has a current position open for a Telecommute Disease Manager II.

Core Responsibilities Include:

• Developing future-oriented goals and health actions
• Advocating for members and families
• Conducting assessments to obtain information for a participant-centered plan of care

Position Requirements Include:

• Ability to travel to provider, facility, employer group and/or member locations
• 3 years’ recent/related experience
• Registered Nurse with active licensure in home state
• Must obtain national certification supporting disease or case management principles
• Ability to operate a personal computer (PC), including proficiency in Microsoft Office Products
• Knowledge of NCQA standards for case management organizations
