Job Summary
A healthcare provider is filling a position for a Telecommute Health and Social Services Care Coordinator in the Johnson County Area.
Candidates will be responsible for the following:
- Assessing, planning and implementing individualized care strategies with least restrictive care
- Managing the care plan throughout the continuum of care as a single point of contact
- Communicating the required health-related information to ensure quality coordinated care
Qualifications for this position include:
- The ability to travel in assigned region to visit Medicaid members in their homes (about 50%)
- Associates degree
- 4+ years of experience working within the community health setting in a health care role
- Experience in long-term care, home health, hospice, public health or assisted living
- 1+ years of experience working with MS Word, Excel and Outlook
