Job Summary

An insurance company has an open position for a Telecommute Health and Social Services Coordinator in the Holton Area.

Individual must be able to fulfill the following responsibilities:

  • Managing the care plan throughout the continuum of care as a single point of contact
  • Identifying and initiating referrals for social service programs
  • Assessing, planning and implementing care strategies

Qualifications for this position include:

  • Expect to spend about 50% of your time in the field
  • Experience in long-term care, home health, hospice, public health or assisted living
  • Associates degree
  • 4 years of experience working within the community health setting in a health care role
  • 1 years of experience working with MS Word, Excel and Outlook
