Job Summary
A health insurance company is in need of a Telecommute Inpatient Services RN Case Manager.
Individual must be able to fulfill the following responsibilities:
- Identifies member's level of risk
- Manages assigned case load
- Conducts a transition discharge assessment onsite and/or telephonically
Position Requirements Include:
- Travel 25 % of the Time
- 4 years of diverse clinical experience in caring for the acutely ill patients
- Case Management Certification (CCM) or ability to obtain CCM within 12 months after the first year of employment
- Current unrestricted Texas RN license or compact license
