Job Summary

A health insurance company needs applicants for an opening for a Telecommute LCSW Case Manager in Jacksonville.

Core Responsibilities Include:

  • Engaging Members telephonically or face to face with a focus on health coaching and member education
  • Planning, implementing, coordinating and monitoring Case Management to include acute and non-acute services
  • Facilitating care for high risk members across the continuum of the health care experience

Required Skills:

  • Up to 30% travel within and outside of the State of Florida
  • Related Bachelor’s degree or additional related equivalent work experience
  • LCSW - Licensed Medical Social Worker - State Licensure Current Florida Licensed
  • 4+ years related work experience. Experience Details: Managed Care, Home Healthcare, and Community Healthcare
  • Required Management Experience
