Job Summary

A health insurance company is seeking a Telecommute LTSS Service Coordinator Field Registered Nurse in the Fredericksburg Area.

Core Responsibilities of this position include:

• Performing face-to-face clinical assessments for the identification, evaluation, coordination and management of members’ needs
• Identifying members for high risk complications
• Managing members with chronic illnesses, co-morbidities, and/or disabilities

Qualifications Include:

• Travel required within assigned territory
• Current, unrestricted RN license in the state of Virginia
• Experience in MS Office suite to include Word, Outlook and Excel
• Minimum of 3 years experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or working with Medicaid, managed care and/or LTSS required
