Job Summary
A healthcare company is seeking a Telecommute MSW Case Manager in Pierce County.
Candidates will be responsible for the following:
- Developing and implementing a case management plan in collaboration with the member
- Conducting telephonic, face-to-face or home visits as required
- Maintaining ongoing member caseload for regular outreach and management
Must meet the following requirements for consideration:
- Local travel of up to 40% may be required
- 1-3 years in case management, disease management, managed care or medical or behavioral health settings
- Social Worker, MSW - Master's Degree
- Must have valid driver's license with good driving record
