Job Summary
A health insurance company needs applicants for an opening for a Telecommute Nurse Care Manager I.
Must be able to:
- Ensure member access to services appropriate to their health needs
- Conduct assessments to identify individual needs and a specific care management plan
- Coordinate internal and external resources to meet identified needs
Must meet the following requirements for consideration:
- Requires 3 years of acute care clinical experience, condition specific clinical experience, home health/discharge planning experience
- 1 year in a Nurse Care manager I role or equivalent experience
- Current, unrestricted RN license in applicable states required
- Must have knowledge of medical management process and ability to interpret and apply member contracts, member benefits, and managed care products
