A healthcare company has an open position for a Telecommute Nurse Case Manager in King County.
Core Responsibilities Include:
- Completing clinical assessments of members per regulated timelines
- Determining who may qualify for case management
- Developing and implementing a case management plan
Applicants must meet the following qualifications:
- Up to 40% travel and must have a valid driver's license and good driving record
- Completion of an accredited Licensed Practical Nurse (LPN) Program
- 1-3 years experience in case management, home health or medical or behavioral health settings
- If licensed, license must be active, unrestricted and in good standing
