Job Summary

A non-profit healthcare system has a current position open for a Telecommute Post Heart Transplant Coordinator in Minneapolis.

Core Responsibilities Include:

• Coordinating and preparing patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team
• Acting as a liaison for transplant physicians to implement medical management of conditions
• Providing telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes

Qualifications Include:

• Ability to attend primary company location, as needed
• BA/BS in Nursing Experience
• 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care
• MN RN license
• BLS Certification
• Ability to assume on-call responsibilities after office hours by providing a resource for medical triage
