Job Summary

A healthcare company has an open position for a Telecommute Registered Nurse Case Manager.

Individual must be able to fulfill the following responsibilities:

• Assessing, planning, implementing and coordinating case management activities
• Facilitating the member’s overall wellness
• Addressing issues to enhance the short and long-term outcomes of member care

Required Skills:

• Ability to travel up to 10% of the time
• 5+ years clinical practice experience required
• RN with current unrestricted state licensure required.
• Desire to work remotely and in-person with members
